Not an expert, but i think in OCD it's more of a compulsive action. Kinda like "It has to be this way or something bad happens".

In my flavour of "autism light edition" (autistic features diagnosis in the 90s) it is more about absolutely hating the way pills feel & taste, so i have to find ways to trick my brain into giving me access rights to the swallow function. Combining it with food usually works too.
